Au sein de la fédération médico-chirurgicale de pathologie digestive du CHU d’Amiens, il a été développé depuis 2001 une prise en charge multidisciplinaire et originale des occlusion colique sur stenose néoplasique. La mise en place d’une prothèse metallique auto-expansible sous contrôle radio-endoscopique permet de traiter en urgence l’occlusion. Ce mode de prise en charge permet de réaliser une chirurgie optimale car non soumise à l’urgence et d’améliorer l’état physiologique du patient dans l’intervalle entre prothèse en urgence et chirurgie éléctive. Nous avons souhaité par ce travail évaluer si ce mode de prise en charge est optimal carcinologiquement. Ce travail est le fruit des efforts conjoints entre radiologues, gastro-enterologues, anesthésistes et chirurgiens.

Summary This science-based book is the first to demonstrate that insufficient access to the sun’s ultraviolet-B (UV-B) irradiance, necessary for photosynthesis of vitamin D, increases the risk of progression in prostate cancer.

At least 80 per cent of our major circulating reservoir of the vitamin, 25-hydroxyvitamin D, is produced by casual exposure of unprotected skin to solar UV-B. The remainder is obtained from foods and supplements.

The author looks at five of the many populations characterized by elevated death rates from prostate cancer: African-North Americans, Norwegians, Swedes, Swiss, and Danes. He shows that insufficient photosynthesis is common to them all and describes the causative circumstances in each instance.

An in-depth review of the literature revealed that a substantial increase in the daily supply of vitamin D recommended for adult North Americans is urgently needed. Introduction

This paper will discuss what is being done in using computers to aid in diagnosis and reading of MRI scans. The primary use of computers in diagnosis is that of a “second reader.” A second reader’s purpose is to validate or verify the first reader, an experiences physician or oncologist determination. We at Kanecki Associates, Inc have developed software that has had medical study evaluation at Wheaton, study 07-010, for validation of cranial cancer areas. The result of the study is that the program is 95% accuracy in diagnosing the significant areas with a 4% false negative rating.

What is computer aided detection (CAD)? Computer aided detection is a method of where the computer is used to read a MRI, PET, CT, or x-ray to make a medical diagnosis. Computer aided detection is referred to as CAD. The primary use of CAD is the cranial, abdominal, and colonoscopy areas.
With a MRI, magnetic resonance imaging, the computer is used to diagnose a patient with cancer in the cranial, abdominal, and colonoscopy areas. The cranial diagnosis uses a method known as the “3 Plane LOC” series. The term “LOC” refers to the different views of the brain as longitudinal, occipital, and cerebral. With the abdominal area, CAD is used to examine the onset of cancer from the external to the internal part of the body. CAD use is based on that of a second reader to validate the original diagnosis. A “second reader” is process whereby the original diagnosis is confirmed. CAD is used in areas of virtual colonoscopy. In the virtual colonoscopy use, the second reader provides another option to a traditional colonoscopy.
CAD is being used in mammography, but some studies do not support its’ use do to greater harm in using CAD for mammogram analysis of cancer. The issue is that CAD for mammogram use has a low success rate below 15%. In addition, CAD for mammography increases the need of biopsies for the patient, according to some studies.
The primary area where CAD is beginning to be used includes cranial, pulmonary, abdominal area, and colonoscopy. In the four areas mentioned, only virtual colonoscopy is where CAD is used as a pre-screen for cancer. In the cranial, pulmonary, and abdominal analysis, CAD is used as a second reader to the primary oncologist evaluation.
Computer aided detection is an emerging technology used in medicine for either as a second reader or a first reader in pre-screening of cancer. Colonoscopy is the leading field of CAD as a first reader, pre-screener.

A reading error is where an expert may determine as area to be positive when it is not, requiring a biopsy, or when an expert determines an area to be negative, when it is positive, delaying the needed treatment of a patient. Another aspect is a false negative in which the patient loses critical time in treatment. A mis-reading can occur even without an expert reader due to various reasons.
