Objective: The main objective of the study is to characterize the epidemiology of needle-stick injuries (NSI) of health care workers (HCWs) working at Hamad Medical Corporation, Doha. Design: This is a prospective descriptive study among the health care workers on the details of needle stick injuries in the year 2001. Setting: 1357-bed three tertiary hospitals of the Hamad Medical Corporation, Doha, Qatar. Subjects and methods: During the year 2001, a survey was carried out among health care workers working in the three tertiary care hospitals. Of the 1274 HCWs who were approached to participate in this study, 1022 workers responded to the questionnaire on needle stick injury with a response rate of 80.2%. Face to face interviews were conducted on the recruited subjects based on a questionnaire that included variables on age, socio-demographic status and immunization history of studied subjects. Results: Of the total studied subjects (1022), 214 studied subjects (20.9%) were victims of the needle stick injuries. 911 HCWs (89.1%) from the total had been vaccinated against hepatitis B (HBV). Higher percentage of sharp injuries occurred among 148 female HCWs (69.2%) than males (30.8%). Percentage of incidence of sharp injuries occurred were more among 10 specialists (25%) and 10 residents (25%) than consultants (12.5%). The highest incidence of episodes of needle stick injuries occurred in the area of specialty such as 28 workers in medicine (13.1%), 20 in laboratory (9.3%) and 19 in emergency (8.9%) followed by 17 in pediatric (7.9%). 195 of 214 HCWs (91.1%) who had sharp injuries had taken HBV vaccination. But, the rest of the 19 HCWs (8.9%) had neglected HBV vaccination. Conclusion: The present study revealed that sharp injuries constitute a considerable proportion of NSI incidents (21%) and could pose the greatest risk of blood borne virus transmission. It could be the reason that the HCWs fully knew of the risk but impediments to prevention of NSIs exist. This could be a topic for further research in the future.

This study assesses the loss of life expectancy and potential working life in Taiwan, between 1986 and 2000, from various types of occupational accidents resulting in mortality or permanent disability. The databases on occupational mortality and permanent disability were obtained from the Bureau of Labor Insurance, with this information then being linked to national death registry data to construct the survival function. A Monte Carlo simulation method was subsequently used to extrapolate the survival rate for up to 600 months, to derive the life expectancy for different types of occupational accidents leading to permanent disability (n=81,249). Based upon the life table for each calendar year, the life expectancy lost by age-gender cohort was also estimated for cases of mortality (n=20,001). In those cases resulting in permanent occupational disability, variations in the expected years of life lost (EYLL) were demonstrated by different occupational injuries, ranging from 7.4 to 13.6 years per case. The overall EYLL through permanent occupational disability is found to be almost identical to that of occupational mortality, with a ratio of 1.04:1. We conclude that permanent disability resulting from occupational injuries has a significant impact on society as a whole.

This exploratory study aims to investigate the associations between some environmental factors and the increased risk of motor vehicle crash-related injuries among taxi drivers. Information utilised in the study are obtained from police reports of all road traffic accidents that occurred on the roads between 1996 and 2000 in New South Wales (NSW), Australia. Of the 7923 taxi drivers who involved in crashes, nearly 10% (n=750) were killed or injured. Results indicate sex, and two environmental factors are significantly associated with an increased risk of crash-related mortality and injury among taxi drivers. The adjusted relative risk of crash-related mortality and injury is increased by 60% for those who work the night shift (OR=1.59, 95%CI=1.35-1.88), and by 20% for those who do not carry any passenger on board (OR=1.20, 95%CI=1.02-1.41) should these drivers involve in a crash. The increased relative risk of crash-related mortality and injury is nearly 2.5 times for female taxi drivers (OR=2.30, 95%CI=1.45-3.65) when compared with their male counterparts.

Objective: To investigate the accuracy of a computerized method for classifying injury narratives into external-cause-of-injury and poisoning (E-code) categories. Methods: This study used injury narratives and corresponding E-codes assigned by experts from the 1997 and 1998 US National Health Interview Survey (NHIS). A Fuzzy Bayesian model was used to assign injury descriptions to 13 E-code categories. Sensitivity, specificity and positive predictive value were measured by comparing the computer generated codes with E-code categories assigned by experts. Results: The computer program correctly classified 4695 (82.7%) of the 5677 injury narratives when multiple words were included as keywords in the model. The use of multiple-word predictors compared with using single words alone improved both the sensitivity and specificity of the computer generated codes. The program is capable of identifying and filtering out cases that would benefit most from manual coding. For example, the program could be used to code the narrative if the maximum probability of a category given the keywords in the narrative was at least 0.9. If the maximum probability was lower than 0.9 (which will be the case for approximately 33% of the narratives) the case would be filtered out for manual review.
